can be revitalized when a patient is asked to put down his/her phone number for
remembrance in case the patient forgets his/her appointment date. There should
be disciplinary actions taken against any patient who fails to be punctual on
his/her clinic days however, when giving time to patients, the record officer
should checkmate the distance, the patient is from to enable them meet up with
time.
