The Listowel Memorial Hospital has been providing acute care and complex continuing care services to the residents of North Perth and the hospital’s catchment area since 1919. Our fully accredited hospital offers a wide range of primary and secondary services including 24-hour emergency services. In-patient and out-patient care, specialist clinics and supportive diagnostic services. The Hospital has an active staff of 10
Family Practitioners as well as several consultant staff.

Registered Dietitian
POSITION POSTING # L12-001 – Listowel Site
Reports to: Manager ER/OR/Ambulatory Care
STATUS: 30 hours per week

Accountabilities:
• Provide Individual and group education sessions
• Utilize health promotion strategies for chronic disease prevention and management
• Provides clinical nutrition and diabetes education services to inpatients and outpatients
• Develops Clinical Program Plans and evaluates on an on-going basis
• Identifies priorities, standards of care, and interventions in order to provide an
individualized treatment plan that achieves optimal patient outcomes
• Integrates Best Practices/Clinical Practice Guidelines accordingly
• Provides community awareness programs and information for general public

Education:
• Registration with College of Dietitians of Ontario (CDO)
• Membership with Dietitians of Canada and CDA-Diabetes Educator’s Section
• Certified Diabetes Educator, preferred

Experience:
• Minimum 1 year post-internship/full-time experience
• Priority Areas: diabetes education, nutrition education/counseling: program planning
