This is a research carried out on the attitude of pregnant women toward
the acceptance of caesarean section as an alternative to vaginal delivery in Central
Hospital Sapele. Caesarean section is the most common surgical procedure done
in obstetrics, and it still remains one of the oldest surgical procedure.

Caesarean section is a operative procedure that is done or performed to
deliver a baby via transabdominal route (Baker 2006).

A caesarean section is usually performed when vaginal delivery would put
the baby’s or the mother’s life or health at risk. Although in well developed
countries, it is performed on request or by choice for childbirth that would otherwise
be natural (Guise, 2004).

The first modern caesarean section was performed by a German
Gynaecologist Ferdinard Adolf Kehrer in 1881.

The Roman Lex caesarean of Numa Pampilus (715-673BC) required that the
child of a dead mother be cut from her womb, this seems to have began as a
religious requirement that mother should not be buried with pregnancy, and this
evolved into a way of saving the fetus with roman practice, requiring a living
mother in her 10th month of pregnancy be cut opened reflecting
knowledge she would not survive the delivery.

Rumours have it that the term “caesarean” was gotten from the Roman
dictator “Julius Caesar, that he was delivered through the procedure although
caesarean section were performed in Roman times, there are no classical source
record of a mother surviving such a procedure. The earliest recorded survival
of a mother dates to 1500AD and Caesar’s mother Aurelia Cotta lived to serve
her son as an advisor in his adulthood, so it was not possible for her to have
had caesarean section and remained alive.

The term has also be explained as deriving from the verb caedo “to cut”
with children delivered this was referred to as Caesories that is “cut from the
womb”

Caesarean section usually resulted in the death of the mother, the first
recorded incidence of a woman surviving caesarean section was in the 1580’s in
Switzerland when the life of the wife of a pig gelgder was saved through this
operation. In early times, the mortality rate was high so it was considered
when the mother is already dead. As at 1865, the mortality rate was 85% in
Great Britain until improved technique were carried out such as asceptic
technique. Blood transfusion, anaethesia and antibiotics therapy reducing the
mortality rate.

Attitude of women toward caesarean section varies in different part of
the world, a study in United States indicates that married white women giving
birth in private hospitals are more likely to have caesarean section than poor
women even though they are less likely to have complication that may lead to
caesarean section being required. The women in this study have indicated that
their preference for caesarean section is more likely to be partly due to
consideration of pan and vaginal tone. In contrast to this, recent study in
British medical journal retrospectively analyzed a large number of caesarean
section are not likely in women of higher social class than women of other
classes.

The attitudes of women who give birth by caesarean section differs
significantly from those who deliver vaginally, the former are less satisfied
with the experience and themselves especially when the operation is not by
personal preference (Kelvin MC 2007), it also showed that the woman’s
expectation is channeled toward a natural outcome of labour when she is in
control throughout and experience a sense of fulfillment of the time of labour.

Green et al. (2002) used a
hypothetical situation to gauge the extent at which women would prefer
caesarean section if advised adequately by the physician, women maintained a
strong commitment to vaginal delivery while only 15% of women preferred
caesarean section and the most common reason given for such a preference where
wish for natural delivery process, a fast recovery after delivery and a safer
mode of delivery.

I was motivated to carry out this research base on my personal
observations during my posting to the hospital, seeing that most women lost
their babies and even their own lives due to late or no acceptance of this
operation.
