REGISTERED NURSE (RN) CASE MANAGER – Assertive Community Treatment Team

The Mental Health Nurse is responsible for conducting psychiatric assessments, assessing physical health needs and making referrals to community services. The RN is also responsible for providing management and administration of medication in conjunction with the ACT psychiatrist.
The case manager also shares the shift management responsibility with other team members working evening and weekend shifts.
SUCCESSFUL CANDIDATE REQUIRES:
* Diploma in Nursing; Degree in Nursing preferred; OR Registered Practical Nursing Diploma
* Minimum 2 years experience in psychiatric nursing, community psychiatric nursing preferred
* Must have Registered License with the College of Nurses of Ontario or if new graduate, must have temporary nursing license
* Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services
* Demonstrated commitment to the Housing First model and philosophy
* Familiarity with the Ontario Mental Health Act and Mental Health reform
* Knowledge of PSR and Recovery as guiding principles for assessing and developing treatment plans for clients
* Demonstrated skill and competence to establish supportive trusting relationships with persons with a serious mental illness with respect for clients’ rights and preferences.
* Participation in daily staff organizational meetings and treatment planning review meetings
* Excellent mental health and addictions clinical mental health assessment/interview skills
* Taking the lead role or participation in providing medication administration and medical services
* Knowledge of psychotropic medications, especially the newer neuroleptics and their intended effects and possible side effects in order to develop, revise, maintain and supervise team pharmacologic and medical treatment and medication policies, including evaluating and charting psychotropic medication effectiveness, complications, and side effects; and arranging for required blood work according to protocol
* Ability to dispense and inject neuroleptics
* Under the direction of the team psychiatrists and in collaboration with other nurses on the team, organize and manage the system of getting medication to clients and integrating medication administration into clients’ individual treatment plan
* Ability to teach clients, families, the COTA Health Recovery ACT team and other staff about medications, symptoms, side effects and health and wellness behaviours
* Under the direction of the team psychiatrists and in collaboration with other nurses on the team, manage pharmaceuticals and medical supplies
* In collaboration with the team psychiatrist, coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment. Awareness of psychotropic medication side effects that can lead to physical health issues, including weight gain and diabetes
* Experience working with clients with a concurrent disorder and those living in unstable housing situations or homelessness
* Demonstrated ability to work independently, cooperatively and constructively within a multidisciplinary team
* environment
* Ability to work with agencies, hospitals and the broader community
* Knowledge of trends and issues related to the delivery of services to a mental health client population.
* Excellent knowledge of community resources available to people living with mental health illness including those related to diversity
* Excellent organizational and interpersonal skills, and the ability to work flexible hours, including weekends and holidays
* Excellent diplomacy and conflict resolution skills
* Excellent computer skills (e.g., Word, Excel, Microsoft Outlook)
* Ability to manage and adapt positively to an ever changing health care/work environment.
* Excellent oral and written skills
* Experience in networking with Mental Health groups/agencies an asset
* Experience working in a community agency an asset
WORKING CONDITIONS
* Visits will be conducted in the clients’ homes, hostels, hospitals and other community settings
* Weekend and evening rotation
* Must have a valid Ontario driver’s license and reliable means of transportation on a regular basis. Access to reliable vehicle for the purpose of work is required
