Quality health records keeping has been having chronic
problems in the health care industry, some identified problems includes, lack
of trained health records officers, poor communication between patient and
health information personnel, inadequate provision of filling equipment e.g.
cabinets/shelves etc. lack of concentration in duty, lack of confidence in the
security of health data may include patient to conceal sensitive information as
a consequence their treatment may be compromised, justices is breached when
persons because of their socio economic class or age do not have equal access
to health information resources and public health, service. Lack of adequate
storing facility etc. the problems of this lies in assessing the effect of
quality health records keeping as hub of health care industry for the provision
of patient care in Oghara Teaching Hospital Delta State.
