For a health record to be able to serve the purpose
for which it is being kept, it must possess some qualities which includes good
documentation should be accurate, it should be legible, it should be simple
avoidance of ambiguity, it should be able to improve services delivery should
improve quality of life, must be able to identify people contributing to the
record so that they can be asked for further information if necessary (Babatude,
2008).
