The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team of providers includes: physicians, nurse practitioners, registered nurses, social workers, health promoters, dietitians and physiotherapists.

Nurse Practitioner
1.0 FTE – 37.5 hours/week
The Nurse Practitioner will provide a range of services including counselling, health promotion and preventative care, comprehensive triage, physical assessment, diagnosis, and treatment. The Nurse Practitioner may also participate in health promotion programs.

Requirements:
• Current registration with the College of Nurses of Ontario as a Registered Nurse, Extended Class and a member in good standing/ new graduates welcome to apply
• Understanding of the Community Health Centre model
• 3-5 years of community nursing experience
• Strong health assessment and organizational skills
• Experience presenting health promotion programs
• Demonstrated ability to function independently within a team setting

Flexible hours of work are required. This position includes benefits. The incumbent will be required to work at the Forest and Kettle Point sites.
