Family Heath Teams (FHTs) use a patient-centred approach to primary health care that brings together physicians, nurses, nurse practitioners, social workers and other health-care professionals to coordinate the highest possible quality of care for patients-when they need it, as close to home as possible. FHTs are a key component of Ontario’s health-care plan to reduce wait time, improve access to physicians and nurses, and keep Ontarians healthy.

SOCIAL WORKER
Do you combine strong clinical skills/knowledge and in-depth experience with mental health in a primary care setting working collaboratively with family physicians and allied health professionals? Are you autonomous, yet able to work effectively with all members of the inter-professional health-care team? Then join us on the front lines of responsive health-care delivery. With your 5 plus years of experience as a social worker, you will act as a patient and family advocate, plus identify and support access to appropriate community, volunteer and professional services and programs. Patient education, assessments, treatment planning and treatment delivery in a number of settings including, individual, family and group therapy sessions will also be among your duties.

Success in this role calls for someone who can explore alternative community supports, utilize evidence based practices along with creative strategies that meet the needs of diverse patient, family and provider populations. You hold a BSW and are registered with the appropriate professional association. The successful candidate has excellent computer skills in software programs such as; Microsoft Outlook, Word and Excel and experience with an EMR. A valid driver’s license and access to a vehicle are needed on occasion. Knowledge of French and a master’s degree are considered assets.
