catchment area of 165,000 through 12 health centers and two hospitals in
partnership with the Ministry of Health.
Abwenzi Pa Za Umoyo (APZU)
is a branch of PIH has worked in the rural Neno District of Malawi
for over ten years, in collaboration with the Ministry of Health (MOH) to
support clinical care at two hospitals and twelve health centers. Since 2014,
APZU has been operating the Integrated Chronic Care Clinic (ICCC), a unique
clinical model that provides longitudinal care for all of a patient’s chronic
conditions, including HIV and/or Non-Communicable Diseases (NCDs).

disease, uncontrolled diabetes, sickle cell, etc). The Advanced NCD Clinic
will be staffed with a clinical team especially trained in the management
of these conditions. This team will be responsible for the routine operations
and clinical care in this clinic as well as for mentorship and support to NCD
care at the primary care level in Neno’s 14 health facilities.

Nurse Mentor will be a core part of the operations, clinical care,
teaching, and research at the two hospital-based advanced NCD clinics and will
helpoversee nurses on a clinical team that will coordinate patient recruitment,
provide longitudinal patient care, and identify patients who need closer
monitoring. This team will also be responsible for providing broad NCD
mentorship in Neno, including for the primary care IC3 system and for
inpatient NCD care at the two hospitals. The NCD Nurse Mentor will work
closely with the NCD Physician Lead to integrate the Advanced
NCD Clinic into the operations of IC3 and the other clinical and community
programs in order to provide holistic care to the patients in the district.
This will include having a presence in the hospitals to help improve the
quality of care of inpatient chronic diseases. Additionally, the candidate will
work with the Neno-based leadership and US-based clinical team to develop
clinical protocols and research projects surrounding NCDs. He/she will also be
a key collaborator with the MOH in Malawi, both locally in Neno and
in Lilongwe. The candidate for this position must be self-motivated, have
superb interpersonal skills, be passionate about nursing care and
social justice, and show creativity in tackling clinical and organizational
problems as they arise.
