Mind Melbourne North Community
Services provides Individualised Client Support Packages to people experiencing
mental ill health in the LGAs of Banyule, Darebin, Nillumbik and
Whittlesea.
Each client receives an
individual funding package tailored to their support needs; these packages can
include outreach support, group work, goal planning, care coordination,
brokerage, housing and support and other personalised support activities.
Staff outreach to family homes,
inpatient units, Community Care Units, SECU wards, prison, public and private
housing. The length of time of support is variable as each package is
established with specific goals and timelines
The Community Mental Health
Practitioner will provide services to clients, families and carers in line with
Mind’s Model of Recovery Oriented Practice and according to the values of Mind.
