NURSE PRACTITIONER

Job Description:
Using advanced nursing knowledge and practice, consultation and effective leadership skills, you will strive as an active member of the interdisciplinary team to improve the clinical outcomes of patients and families serviced on the Restorative Care Unit.
Assist in admission and ongoing management of patients admitted to the restorative care unit. Assist in development of the program’s clinical guidelines and protocols, promoting the use of research.
Education:
• RN (EC) with College of Nurses of Ontario require – NP Adult
• Masters degree in Nursing with a current College of Nurses of Ontario registration
• Acute Care NP with minimum 2 years experience in Rehabilitation, Restorative and/or Complex Continuing care preferred
• Knowledge/experience of rehabilitation/restorative care, falls prevention, chronic disease management, dementia care, and other geriatric-related care preferred
• The NP will provide primary care management within the restorative care hospital setting
• Demonstrated excellence in attendance required
• Excellent interpersonal skills and ability to work effectively in a non disciplinary team
• Knowledge of and adherence to Occupational Health & Safety and infection control principles.
• Must have current valid driver’s license and vehicle available for use or transportation arrangements appropriately available as assignment dictates
