Ontario Canada Nursing Jobs Vacancies in Bramalea Community Health Centre

Bramalea Community Health Centre (BCHC) is now recruiting two Nurse Practitioners; one for its Bramalea site and one for its Malton. BCHC is a non-profit, values based organization that provides health promotion and prevention programs, primary health, oral health care and works with local residents and other agencies to increase community capacity. BCHC is seeking a dedicated team player with plenty of initiative, creativity and exceptional communication, presentation and relationship-building skills. Nurse Practitioners (RNEC)
Specific Responsibilities:
• To provide holistic care and to act as an entry point into the broader health or social service system.
• To provide health care and treatment plans for client with minor episodic illnesses, minor injuries and stabilized chronic illnesses as well as clients presenting with complex health and social needs
• Participates in the development, implementation and evaluation of treatment, education, counselling and health promotion services for individuals, families and the community.
• Participates in the ongoing development of the clinical department and the centre as a whole
• S/he is an active member and contributes on a number of interdisciplinary teams.
• S/he works in the health centre and in outreach locations in accordance with BCHC’s service plan

Qualifications:
• Baccalaureate Degree in Nursing with current extend class certificate of competence from the College of Nurses of Ontario.
• Member of the Registered Nurses Association of Ontario and the Nurse Practitioner Association of Ontario.
• Advance knowledge and proficiency in current, evidenced-methods and practices of primary care delivery with an emphasis on health promotion, risk reduction and injury prevention.
• Ability to work independently and collaborate effectively in an interdisciplinary team environment.
• Excellent communication skills, both verbal and written, along with the ability to prioritize and multi-task.
• Familiarity with client centered and self-management approaches to chronic disease prevention and management.
• Proficient in the use of computers including Word, Excel, PowerPoint and electronic health records.
• Previous experience in community health/social services and/or a values based organization is an asset.
• Languages relevant to Bramalea and Malton residents a definite asset.

The successful candidate will demonstrate a willingness to participate in BCHC’s service delivery philosophy and commitment to becoming an inclusive, barrier free health centre and an ability to work interdependently and respectfully in an interdisciplinary team.

This position requires flexibility in hours of work to support accessible program service delivery and the centre hours of operation. As such the position entails some evening and weekend work as well as on call.
Compensation: $74,038 – $89,203 with HOOPP pension plan
