Duties and Responsibilities
• Provide primary care within the scope of the practice of Nurse Practitioner as outlined in the College of Nurses of Ontario Standards of Practice for Registered Nurses in Extended Class. The provision of primary care includes: assessments, diagnosis, counseling, screening, referral, education, treatment and follow-up.
• Collaborate and work effectively with Physicians in two practice locations to facilitate appropriate provision of primary care outside of scope of practice.
• Maintain complete, accurate and timely records of patient visits using EMR
• Liaise with physicians, EWFHT interdisciplinary staff team members and community agencies as required to help improve the team’s ability to provide a broader range of services.
• Provide individual and group sessions to patients to educate and help them manage particular health issues or chronic disease.
• Prepare statistical reports for the Ministry of Health on a quarterly basis.
• Will work in two clinical locations and must be available for after hours clinical work one evening per week
Professional/Education/Experience:
• Registered Nurse Extended Class (EC)
• Baccalaureate or Masters in Nursing
• Current registration with the College of Nurses of Ontario
• Current registration in professional associations, RNAO, NPAO
• Experience in a community health care setting and program development and facilitation.
• Must have access to a vehicle, a valid driver’s license and adequate insurance.
• Excellent oral, written and clinical skills
• Ability to work effectively with all members of the health care team
• Knowledge and proficiency in current, evidence based methods and practices of primary care delivery with an emphasis on health promotion and disease prevention.
• Proficient in information technology.
• Ability to prioritize, manage time effectively and be a flexible team player in growing FHT.
