Community Nurse/Support Worker

Through a network of community based centres as well as live-in residential facilities and home support, MS Australia – ACT/NSW/VIC and its dedicated volunteers and staff are an essential element assisting Australians living with MS to maintain their quality of life

We now seek a Registered Nurse to join our experienced & highly respected Client Services team. You will be responsible for providing information, support and nursing services to address the needs of people with MS in the Gippsland Region of VIC. This is a unique opportunity to use your broad range of skills and to make a difference in the lives of people living with MS!

Your duties will include:
• Providing clinical nursing assessment, management advice and review of clients with progressive neurological conditions, including MS
• Providing immunotherapy support, including training and education in the use of disease modifying drugs and self-injection techniques, side effect management and individual follow up
• Promoting and supporting client self-management by adopting a person centred approach to all client interactions, and empowering clients to actively participate in treatment planning and goal setting
• Facilitating the implementation of Individual Program Plans (IPP) by identifying, coordinating, promoting and making referrals to appropriate internal and external services
• Monitoring, measuring and evaluating IPP outcomes using evidence based tools

We seek a self-motivated individual with:
• Experience in providing nursing services in a clinical setting
• Ability to identify and address psycho-social needs
• Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
• Strong interpersonal, verbal and written communication skills
• Demonstrated ability to work autonomously with minimal supervision
• Intermediate computer literacy skills, particularly in Microsoft Office applications
• Current driver’s licence
