Opportunities are now available for a Nurse Practitioner and Registered Nurses to work as part of inter-disciplinary teams led by family physicians in the Hamilton area.
The Hamilton Family Health Team serves as a focus for chronic disease management, community-based health promotion, and disease prevention programs and activities. You will work within the full scope of practice in a patient-centred model of care with a team of allied health care professionals including dietitians, mental health counsellors, and pharmacists. You will have the opportunity to participate in professional development through workshops, courses, seminars, and networking with nursing colleagues across the Hamilton area.
The ideal candidate has excellent health assessment/clinical skills, the ability to Perform the full range of duties and responsibilities, and experience in a variety of clinical settings. You also have direct primary care experience (preferred), strong oral and written communication skills, and excellent analytical, problem-solving, negotiating, and computer abilities. Current certificate of registration from the College of Nurses of Ontario as an RN or RN-EC is required.
