Conclusion

Health records have the potential to improve the
quality, safety and efficiency of care when fully implemented. The information
contained in the medical records allows health care provide informed patients
medical care. The health records serve as the clinical record or health record
are the most important and basis of clinical tools aggregated to form a
permanent of individual consideration and the reasons for decision making. It
is essential for effective communication and good clinical care, but they are
often accorded low priority, poorly maintained, not readily available,
independent inquires etc.

This study draws together standard and concluded with
some good practices points for a fit-purpose structure, multidisciplinary
records to support good care and protect the interest of patients, health care
industry and clinicians. This principle should be equally applicable to
electronic health records keeping.
