An opportunity is available for an experienced health professional from an Allied Health or Nursing (Division 1) background to join the Transition Care Program as a Care Coordinator.
The successful applicant will be based at The Northern Hospital and work throughout the community to provide case management to a small caseload of clients. The successful applicant must be able to work independently across the acute and community interface.
The ability to perform patient assessments, care planning, a commitment to teamwork and the ability to work in both the hospital and community settings are required. Excellent communication skills, including the capacity to negotiate effectively, the ability to work in a fast paced environment and to establish priorities are essential components of the role.
A genuine interest in providing optimal outcomes when supporting aged clients living at home is required.
