The Etobicoke Medical Centre Family Health Team is a collaborative health care team, committed to providing excellent, evidence-based, comprehensive care to our patient community. The EMCFHT, funded by the Ontario Ministry of Health and Long-term Care, is in its second year of operation and is built upon a longstanding medical practice. The physicians and staff of the Etobicoke Medical Centre have been providing high quality care, including prevention, treatment and diagnostics, to the community in the west end of the city of Toronto and Mississauga for over 40 years.
Nurse Practitioner – Primary Health Care (Registered Nurse – Extended Class)
The Etobicoke Medical Centre Family Health Team (EMCFHT) is looking for a motivated and dynamic Primary Health Care Nurse Practitioner (NP) with excellent interpersonal, clinical and counselling skills to join our Interprofessional Team. The ideal candidate is an innovative, self-starter who is client-focused and is committed to providing accessible comprehensive primary health care. This candidate will work collaboratively with Physicians, Nurse Practitioners and the Interprofessional Team to ensure patients receive primary health care services within the full scope of practice as RN (EC).Qualifications
Education
Registered as a RN-EC, Primary Health Care, in good standing with the College of Nurses of Ontario
Valid BCLS
Masters of Health Science in Nursing or Masters in Nursing

Clinical Experience
Minimum of five years’ experience in community and /or hospital setting required; experience working with a geriatric population preferred
Minimum 2 years experience as a Primary Care NP preferred
Experience working in a collaborative care model or an interdisciplinary team
Experience in program planning, implementation and evaluation; preparation and/or selection of education materials/programs

Skills
Demonstrated strong clinical skills; including experience in assessment, counselling, developing and monitoring care plans.
Ability to work independently and collaboratively as part of an Interprofessional Team
Ability to integrate new knowledge into an evidence-based practice
Understanding of and commitment to continuous quality improvement
Excellent communication and interpersonal skills, ability to work as part of an Interprofessional Team and independently
Proficient computer skills: clinical documentation and communication in EMR – Practice Solutions; Microsoft Office Suite
