Our multi-disciplinary approach includes the provision of primary health/dental care, counselling, harm reduction, community development, advocacy, and innovative partnerships with other organizations.CTCHC is a leader in the development of inner-city health services: Shout Clinic focuses on the needs of homeless and street involved youth under 25; Queen West focuses on adults, families, and youth in the local area. We currently have an opportunity for a NURSE PRACTITIONER
Extended Class Nurse – 0.8 FTE (Peri-natal and Primary Health Care)
Compensation: Salary – $ $74,038 – $89,203 pro-rated to 0.8 FTE $59,230 – $71,364

Responsibilities:
* Provide pre-natal and post-natal health care to women and babies
* Provide health information and counseling to women and their babies
* Provide primary health care through clinical assessments, treatment, planning, implementation of care and evaluation on site
* Functioning both independently and within a multi-disciplinary team model

Qualifications:
* Member in good standing with the Ontario College of Nurses. Current registration with the Ontario College of Nurses (Extended Class)
* Clinical experience in prenatal and post-natal primary health care
* Clinical experience working in a primary health care setting
* Knowledge and awareness of issues related to homelessness, immigration, mental health, poverty and discrimination
* 3-5 years experience in a community based setting working with marginalized groups
* Knowledge of the health care/social service systems and resources available to clients
* Strong communication, problem solving and conflict resolution skills
* Ability to manage complex workload and remain flexible
* Fluency in languages other than English is an asset
