the post applied for and its reference number specified against the vacancy.
Photocopies of academic certificates, transcripts, pass slips and Registration
Certificates, annual practicing licenses where applicable with (3) recent
passport size photographs must be attached on all three (3) Forms. All
applicants should indicate day time phone contacts.
