An exciting opportunity exists for an enthusiastic clinician with an interest in Recovery, Rehabilitation and Intensive Case Management to work in the Hawthorn Mobile Support Team (MST). St Vincent’s is currently introducing the ‘Strengths’ Model of Case management, this model emphases client directed care in the setting of a collaborative relationship.
You are a talented and motivated professional, exhibiting exceptional communication, interpersonal and organisational skills as demonstrated in patient examinations and your relationships with colleagues.
You are a clinician interested in working in a supportive team to provide intensive case management to a small group of clients. The team also provides clinical and case management supervision and support processes.
To be successful in this role you will be degree qualified. Post -graduate experience in mental health, Dual diagnosis and/or rehabilitation experience or knowledge is also desirable.
